This version is a PDF version that can be viewed in different readers. It does contain fillable forms, a pulldown menu, and radio buttons. Viewers of the video can follow along with this booklet on a computer and type their answers and use a mouse to select the answers in the booklet.

For those who wish to print the booklet out as a paper booklet, the pages have been rearranged in this file to be printed out in booklet order (imposed) so that once the pages are stacked and folded, the booklet is created. If you have access to a duplex printer (both front and back of the page printing) then you may be able to print the booklet out directly. If you do not have access to a duplex printer, then you still may be able to print the booklet out on both the front and back by printing the odd pages first, reinserting the paper into the printer, and then printing the even pages. Test printing with your individual printer may be required to make sure all settings are correct.

For those who do not have a duplex printer or cannot get the imposed version to work properly, a print version is included with the pages in their normal reading order. The pages can be printed in reading order as they are and then stapled together, shrunk by your printer to include two pages on each 8.5 x 11 page, or even printed as a booklet (imposed) by using the “Booklet” feature in “Page Sizing & Handling” in Adobe Acrobat or Adobe Acrobat Reader (if available). Test printing with your individual printer may be required to make sure all settings are correct.
